There are two major classes of herniation: supratentorial and infratentorial. Supratentorial herniation is of structures normally above the tentorial notch and infratentorial is of structures normally below it.[4]

  • Supratentorial herniation
  1. Uncal (transtentorial) NOTE: TEMPORAL LOBE MOST FREQUENTLY INVOLVED
  2. Central
  3. Cingulate (subfalcine)
  4. Transcalvarial
  • Infratentorial herniation
  1. Upward (upward cerebellar or upward transtentorial)
  2. Tonsillar (downward cerebellar)
